Information on Obtaining an Accident Report
Copies of the Indiana Officer's Accident Report Forms are available upon request. If you were involved in an accident which was investigated by the Swayzee Police Department and would like to obtain a copy of the written report, you may do so for a fee of $5.00. You may purchase the copy at the Swayzee Police Department which is located at 213 South Washington Street. Or, you may send a self addresses stamped envelope along with a check or money order payable to the "Swayzee Police Department" for $5.00 to the following address:

Attn: Accident Report Request
You need to have the accident number and the names of all parties involved enclosed with your request. The Office is open Monday thru Friday from 9:00 am until 3:00 pm
